DEBIT ORDER AUTHORISATION FORM

I/We hereby request Avianto Private Schools to draw against my/our bank account, wherever it may be held, the amounts becoming due and payable from time to time in terms of the Avianto Private Schools agreement referred to hereunder.
I/We authorise my/our bank, whichever bank it may be, to debit my/our account with any amount which will be drawn by Avianto Private Schools against my/our account in terms of this debit order authority.
I/We understand that withdrawals hereby authorised will be processed by computer through a system known as ACB Magnetic Tape and I/we also understand that details of each withdrawal will be recorded on my/our bank statement.
I/We agree to pay any bank charges relating to this debit order authority and its implementation.
I/We agree that I/we may only cancel this authority by giving Avianto Private Schools at least one calendar month’s prior written notice. As this debit order authority is a requirement of the Avianto Private Schools agreement to which it relates, Avianto Private Schools will always be entitled, in its discretion, exercised reasonably, to refuse to accept such notice of cancellation or require a notice period longer than one calendar month before such cancellation becomes effective.
I/We acknowledge that Avianto Private Schools may not cede or assign any of its rights in terms of this authority to any third party without my/our prior written consent, which may not be unreasonably withheld or delayed, and that I/we may not delegate any of my/our obligations in terms of this authority to any third party without Avianto Private Schools prior written consent.
